DIRECTIONS FOR COMPLETING THE COMMUNITY AGENCY SUPPORT PROGRAM (CASP) APPLICATION
Documents may be opened from the City of Fort Myers website, but not saved there. Documents numbered 1) through 5) below may be emailed to Margo Arcand, Senior Budget Analyst at: marcand@cityftmyers.com.
The application documents should be organized in the following manner:
- The application cover (Word)
- The application Table of Contents (Word)
- The Application (Word)
- The Demographics Summary (Excel) is a separate document from the Application because it is written in Excel, but it is the third section of the application as identified on the Table of Contents.
- Application Forms A & B & C (Excel) are in a separate document because they are written in Excel, and each form is identified by a tab at the bottom left margin of the Excel workbook. When the document is opened, Form A is usually visible, and to access the other two forms, you must left click on each of the tabs to the right of the tab labeled “FORM A.”

These informational documents should be read prior to, and in concert with, completion of the application. The Program Guidelines contain definitions and a Glossary of Terms that are helpful in understanding what data is being requested.
In completing the forms written in Word, additional space may be used. In completing the forms written in Excel, please use only the spaces provided. Please enter the name of your agency at the top of every page. Alternatively, the forms may be printed and completed manually for submission.
